Practicum Activities Reflection:

Over weeks 7 and 8, my preceptor and I reviewed the facility’s readmission rate in the past year and compared it to the previous years. We noted a steady rise in readmission rates over the years, with an increase of 3.2% in the past year. Besides, we identified that most of the readmissions were from cardiovascular diseases, including history of stroke, myocardial infarction, and heart failure. We found that patients with cardiovascular diseases had the highest healthcare spending and worst health outcomes based on high morbidity and mortality rates in the facility. We sought to establish the possible causes of the high readmission rates among patients with cardiovascular diseases through a survey based on these findings. The survey entailed inquiring from nurses on their perceptions concerning the high readmission rates in this patient population. From these activities, I learned that a nurse leader has a major role in promoting high quality of care and patient safety in their organization (Xing et al., 2020). Leaders also have a role in identifying the root cause of problems in patient care delivery.

Leadership is described as a person’s ability to influence others. Influence is considered an essential leadership quality as it enables the leader to move a person or a team towards a certain course of action (Cakir & Adiguzel, 2020). The preceptor influences the work environment using vision and effective communication. The nurse leader regularly communicates the facility’s vision to the nursing staff, which influences them to put effort towards achieving the vision. Communicating the vision signifies to the nursing team that the leader is motivated and is planning for the organization’s future. Besides, the leader has mastered the art of communication, which has been a key element in influencing the nurses in the facility.   Influence is a key tool in protecting and valuing employees in the work environment since the preceptor motivates nurses to endeavor to create a healthier work environment (Cakir & Adiguzel, 2020). Besides, influence is a tool for motivation and inspires employees to perform to their best, making them feel valued.

Practicum Project Preparation

The practicum project focuses on reducing the readmission of CHF patients. It will involve identifying factors that increase readmission rates among CHF patients and establishing interventions that can help mitigate or eliminate these factors (Xing et al., 2020). Potential barriers in the project’s implementation include inadequate cooperation from patients and their families since they are central to its success. Patients may fail to comply with the post-discharge plan’s health education recommendations, resulting in complications and increased readmissions. Besides, low literacy and health literacy levels among CHF patients may limit them from effectively utilizing the recommended health education interventions. It will thus be essential for nurses to evaluate a patient’s health literacy to tailor the education plan to their needs.

Leadership Video Reflection:

The issue of headship video differentiates between heading and leading. I learned that headship is an individual’s position in the official hierarchy. Influence is a key element in leadership.  A person can head a team or an organization but lack influence over their subordinates. Conversely, a person with no position in the hierarchy can lead a team because they influence.  In addition, the video enlightened me that a leader cannot inspire people towards a course without influence. However, power is vital in leadership. It is difficult for a leader to influence individuals towards a common course goal if they do not exercise their power.
