Nursing practice is multidimensional since multiple interventions are used to enhance outcomes. At the federal and state level, different bills and laws are formulated to influence nursing practice differently. Their focus areas vary due to the inevitable differences in healthcare issues and their magnitudes. Understanding how these laws and bills can elucidate their influence on healthcare delivery at different levels.
At the federal level, the Hospital Readmissions Reduction Program (HRRP) influences healthcare organizations to focus more on safety and quality metrics. The HRRP is an Affordable Care Act regulation that prompts the Centers for Medicare & Medicaid Services (CMS) to deduct payouts to healthcare organizations with high patient readmissions (Gai & Pachamanova, 2019). From a patient perspective, readmission represents a repeat patient admission within 30 days. To avoid payout deductions, healthcare facilities improve communication and care coordination efforts to ensure patients and caregivers engage each other during post-discharge planning (CMS.gov, 2022). At the state level, nutrition for inmates can perfectly illustrate how laws influence health practices and outcomes in Maryland. Del. Terri Hill proposed the bill to oblige the secretary of public safety to partner with the health secretary to update the minimum mandatory standards for food services among inmates (Maucione, 2023). Doing so would improve inmates’ health and reduce their vulnerability to chronic diseases like diabetes and liver disease.
These bills/laws also influence nursing practice at the organizational level. In particular, my organization must adhere to the principles of high patient safety and care quality to prevent readmission rates as much as possible. Preventing them ensures that the CMS does not reduce the facility’s payouts due to high readmissions. The nutrition for inmates provides a background for educating patients on healthy eating to reduce vulnerability to chronic diseases. It shows that better health outcomes can be achieved through healthy eating hence the need for partnering with community organizations promoting such programs.
