Patients from racial/ethnic minorities and those with lower SES often have poorer rates of medication adherence (Salmasi et al., 2020). The case study presented reports of a patient with poor blood thinner effectiveness after two weeks of therapy. The patient is a Filipino, one of the low socioeconomic status ethnic communities in the United States. There are other several potential reasons why the medications are not working as intended. To begin with, the patient could not be taking them exactly as prescribed. Blood thinners should be taken exactly as directed to avoid serious side effects. Second, the patient’s diet may have an impact on how effectively the medication functions. Omega-3 fatty acid-rich foods like fish and tofu can increase your risk of bleeding (Al-Saikhan, 2020). Thirdly, it’s possible that the medication is being metabolized incorrectly by the patient’s body due to other medical conditions or concurrent use of other drugs. This may also be caused by genetic factors or other medical problems.

A nurse must build rapport with a patient to take their history effectively and to construct an effective therapeutic relationship (Ball et al., 2019). Case Scenario: Mono Nu, a 44 year-old Filipino patient comes to the clinic today to have his “blood thinner” labs drawn since he started them two weeks ago. Consider how you would build a health history for the patient. What questions would you ask, and how would you frame them to be sensitive to the patient’s background, lifestyle, and culture? Think about the challenges associated with communicating with patients from a variety of specific populations. What strategies can you as a nurse employ to be sensitive to different cultural factors while gathering the pertinent information?
