Health Assessment

Patients from racial/ethnic minorities and those with lower SES often have poorer rates of medication adherence (Salmasi et al., 2020). The case study presented reports of a patient with poor blood thinner effectiveness after two weeks of therapy. The patient is a Filipino, one of the low socioeconomic status ethnic communities in the United States. There are other several potential reasons why the medications are not working as intended. To begin with, the patient could not be taking them exactly as prescribed. Blood thinners should be taken exactly as directed to avoid serious side effects. Second, the patient’s diet may have an impact on how effectively the medication functions. Omega-3 fatty acid-rich foods like fish and tofu can increase your risk of bleeding (Al-Saikhan, 2020). Thirdly, it’s possible that the medication is being metabolized incorrectly by the patient’s body due to other medical conditions or concurrent use of other drugs. This may also be caused by genetic factors or other medical problems.

A nurse must build rapport with a patient to take their history effectively and to construct an effective therapeutic relationship (Ball et al., 2019). Establishing this trust involves recognizing the patient’s unique cultural perspective. The fact that English is not the patient’s first language must also be taken into consideration; therefore, the use of basic terminology is essential when taking a patient’s history. Patients need to feel like you understand them and don’t pass judgment. I’ll endeavor to answer questions honestly and plainly while putting them at ease. It’s crucial to discuss delicate subjects like weight, lifestyle, and major medical concerns in the context of a larger health picture (Dains et al., 2019). I’ll ask the following questions because the patient’s poor blood thinner effectiveness could be caused by other factors:

1. Have you ever tried blood thinners other than the current therapy?
2. Do you have any more medical conditions?
3. Do you use any other medications?
4. Have you been taking any traditional medicines or natural remedies?
5. How’s your diet going?

Case Scenario: Mono Nu, a 44 year-old Filipino patient comes to the clinic today to have his “blood thinner” labs drawn since he started them two weeks ago. Upon assessing the labs the nurse practitioner notes that he is still out of range. When assessing the patients compliance both stated that he had been taking them just as prescribed. He has been doing well and eating a diet rich in fish and tofu. He doesn’t understand why his medications are not working.

Reflect on the specific socioeconomic, spiritual, lifestyle, and other cultural factors related to the health of the patient assigned to you.
Consider how you would build a health history for the patient. What questions would you ask, and how would you frame them to be sensitive to the patient’s background, lifestyle, and culture? Develop five targeted questions you would ask the patient to build his or her health history and to assess his or her health risks.
Think about the challenges associated with communicating with patients from a variety of specific populations. What strategies can you as a nurse employ to be sensitive to different cultural factors while gathering the pertinent information?
