Salmond and Echevarria (2017) observe that nurses are uniquely placed to support and lead the change in healthcare as they provide the link between physicians and populations, especially advanced practice registered nurses (APRNs) like emergency room nurse practitioners. One needs to complete rigorous accredited postgraduate programs in their specialty to become nurses in the emergency room. The essence of this paper is to discuss the roles of emergency room nurse practitioners, practice scope, the regulatory framework that governs the specialty, accreditation entities, and the nursing professional organizations that they can be members. The paper also addresses a controversial issue associated with the practice of emergency room nurses.

Future Scope of Practice for ER Nurse

The enactment of the Affordable Care Act in 2010 and other federal provisions implore healthcare organizations and providers to offer emergency care to patients presenting with different conditions in emergency room settings. These new laws and requirements imply that the scope of practice for emergency room nurses (ERNs) will expand as they are frontline care providers in different situations. More people than ever before continue to access healthcare services from the emergency room, implying that the nurse practitioners in this specialty need more training and possession of effective skills. Emergency room nurses (ENPs) as specialty providers and licensed health care workers who offer services to patients in ambulatory, urgent, and emergency facilities (Trisyani & Windsor, 2019). The ENP assesses, diagnoses, and manages episodic conditions, harms and acute exacerbations of chronic illnesses like hypertension and diabetes. These nurses work mainly in emergency departments (EDs) of their facilities. The American Academy of Emergency Nurse Practitioners (AAENP) asserts that the emergency nurse practitioner role is distinct as it spans populations and the acuity continuum (AAENP, 2021). The organization advances that emergency care focuses on possible life-threatening situations irrespective of a patient’s initial complaint or reason for seeking care. These practitioners are positioned to offer primary care, acute resuscitation, manage complex conditions, and unstable conditions for all patients of all ages.

Regulatory, Certifications and Accreditation Agencies

The regulation of nurse practitioners, including emergency room nurses, is vested in respective state boards of nursing. States have different regulatory structures for APRNs. Over 25 states allow nurses to practice with full authority, while others have restrictive and reduced practice scope. The American Nurses Credentialing Center (ANCC) remains the main organization that will accredit the ENPs alongside getting professional approval from specialty bodies like the AAENP.

Professional Nursing Associations

Professional organizations and associations are essential for nurses as they provide a voice and platform to express their views, network with others, and learn new trends in healthcare provision. The challenges with disparate regulatory structures imply that nurses need some common position on issues that impact their profession, especially the ability to practice to the full level of their training and education (Boman et al., 202). Policy formulation, by nature, is a political process that requires a voice and a common position. Therefore, nurses, including ENPs, should come together through professional associations to champion their courses and get the right forum like the American Academy of Emergency Nurse Practitioners (AAENPs). The associations allow nurses to lobby for policy changes that include granting full practice mandate in states with reduced restrictive practice laws. Professional associations allow nurses to exchange professional ideas aimed at improving care provision.

The three most essential professional organizations for ENPs include the American Academy of Emergency Nurse Practitioners (AAENP), the American Association of Nurse Practitioners (AANP), and the American Nurses Association (ANA). The AAENP is an association that promotes high-quality, EBP for nurse offering emergency care for patients of various ages and severity through collaboration with interdisciplinary teams (AAENP, 2021). The organization creates guidelines for quality and safety in emergency situations, implores its members to continue advanced education, support training in emergency, facilitate research, and collaborate with professional health entities and academic institutions. The AAENP’s vision is to be a specialty entity that is an expert and unifies nurses’ voice in the specialty.

The second professional organization is the American Association of Nurse Practitioners (AANP). The organization offers professional support and encourages members to attain fellowship status for those who have done something remarkable to the profession. The AANP’s mission is to enable nurse practitioners in advancing quality health care by practice, education, advocacy, research studies, and leadership (AANP, 2021). The association provides nurses with a unified voice and network to advocate for their special issues, like expanding the scope of practice at the state level (Rossiter et al., 2017). The third most critical organization is the American Nurses Association (ANA). The ANA is a pioneer nursing association that offers practice standards to help nurses make practice decisions and informs nurses of the recommended scope of practice, including emergency care nurses. The ANA’s mission is to be a leader in the profession in designing the future of nursing and healthcare.

I want to join the professional organization is the American Academy of Emergency Nurse Practitioners (AAP). My worldview as a nurse is to offer care based on respect, dignity, and equality since all people are equal before God. Further, individuals are born free and have inherent dignity. Therefore, being a nurse means understanding the different kinds of patients they care for and interacting with from a professional perspective. The AAENP is a specialty association that seeks to provide quality and safety in emergency room, facilitate research and encourage collaboration (AAENP, 2021). The vision of this association aligns with my worldview as it encourages expert and unified voice to advance the nurse practitioner’s ability to offer care in emergent situations.

Membership in this organization will enhance my practice as it encourages education and training to ensure that emergency nurse practitioners have the required skills to offer quality care. The association will enhance my practice by offering a platform to learn new trends in care provision for emergency room situations. The association will also help network with other professionals through collaboration and research to enhance my practice.

Controversial/ Evolving Issue

The scope of practice authority laws at the state level remains a controversial issue that may affect care provision among emergency nurse practitioners. Over 25 states now allow their nurse practitioners to practice to full scope and authority based on the qualifications. However, nurses in the remaining states still require physician agreements and supervision and are not recognized as primary care providers (Trisyani & Windsor, 2019). Consequently, they cannot practice fully to save lives, especially for patients presenting in emergency rooms and departments. The limited scope of practice makes emergency room nurses in a dilemma when faced with emergent care needs since they cannot order tests and other procedures for a patient. Expanding roles require nurse practitioners to be recognized as primary care providers to supplement the current physician shortage and help enhance care, especially due to the increasing demand for care services. The implication is that nurse practitioners cannot offer quality care in emergent situations when their scope is limited due to existing legal frameworks at the state level (Campo et al., 2017). The issue will affect my future practice since it implies that I cannot provide care to the full level of my training and education in states with reduced or restrictive laws.

Conclusion

Emergency room nurses are part of the emergency room nurses who are critical care providers due to the life-threatening situations that they handle. Imperatively, their scope of practice should be expanded because of the increasing demand for care across the health and age continuum. Professional associations like the AAENP provide a platform for these practitioners to express their concerns and lobby for policy changes in care provision. Nurses gain more knowledge and networks to improve the quality and safety of care provided in acuity situations through these entities.
