The Agency for Healthcare Research and Quality (AHRQ) care coordination is a deliberate process that entails organizing patient care efforts and sharing information among the different providers involved in patient treatment. The primary goal of care coordination is to develop a common goal and strategy to implement evidence-based practice (EBP) interventions to attain safe and effective care (Chen et al., 2019). Through a coordinated care plan, providers identify and focus on patients’ needs and preferences earlier and communicate to the right parties at the most appropriate time.

Seamless transitions among medical professionals lead to increased efficiency in the planning of care and activities to attain the safe provision of services. The present healthcare environment implores healthcare professionals, including nurses, and organizations to adapt and change to align with the evolving patient and population needs, reforms, and medical complexities (Harrison et al., 2021). A core change aspect in healthcare today is improving patient safety which implores providers to devise strategies to assist organizations to adapt through continuous and lifelong learning and development. Therefore, nurses can attain change when they are incentivized with growth opportunities and share information on different aspects of care delivery to reduce and prevent medication errors and other adverse events like falls.

The next change aspect is the conglomeration of health organizations as they merge to create large medical and healthcare groups to expand care and meet reforms. The formation of large hospitals may affect patients as it means that they can bring services closer at affordable costs based on the different care plans that they present. These entities will also employ more advanced and specialty nurses to offer better care to patients with different conditions and help meet their needs (House et al., 2022). While mergers and acquisitions may rise uncertainties in certain areas of care provision, they still offer patients benefits because of the array of services that will emerge out of these establishments.

The other aspect of change is effective communication that allows healthcare providers to enhance their interactions with patients and promote better patient experience, minimize conflicts in care preferences, and creation of common goals through personalized care planning. Further, the use of change aspects attributed to change models like Kotter’s eight-step change process impacts the delivery of change in healthcare organizations (Strom‐Gottfried, 2019). The use of change coalitions means that change can only occur through teamwork. Coalitions and teamwork ensure that a proper flow of change information occurs to improve overall interactions between patients and providers and ensure that patients understand the goals of their treatment plans.

Rationale for Coordinated Care Plans Based on Ethical Decision Making

At the core of coordinated care is the collecting and sharing of patient health information (PHI). Under the Health Insurance Portability and Accountability Act (HIPAA) rules, the protected health information (PHI) rule mandates providers to ensure that patients’ information is not disclosed without their express consent and to authorized professionals. HIPAA focuses on improving interoperability among providers and organizations. However, its privacy rule poses ethical dilemmas for nurses who wish to enhance care provision. The implication is that nurses and other providers should not undermine the privacy and confidentiality of the patient. Nurses’ Role in Care Coordination

Nurses play a critical role in the coordination and continuum of care because they are directly involved in the provision of patient care. Nurses identify patients’ care coordination needs and align them with existing resources and healthcare teams and expertise. Nurses also create different plans of care that allow physicians and other providers to offer patient-centered care based on patient preferences and needs (Brooks et al., 2019). Distinct plans for different care needs and a continuum of care are essential in effective care coordination for nurses and other providers.

Care coordination is a core part of providing patient-centered and evidence-based care for patients with diverse needs and preferences.
