Objectives

I have achieved most of my goals from the practicum experience. Some of the goals that I can confidently consider to have achieved include  accurate documentation of nursing care plans and interventions, collaborating with interprofessional teams in the provision of patient care services, and developing appropriate diagnoses and treatment plans for patients suffering from mental health disorders. I am yet to achieve the desired competencies in some of the objectives. They include resolving ethical dilemmas in my practice, delivering psychoeducation optimally to the patients, and using the different screening tools for mental health problems. As a result, I intend to work proactively with my preceptor to achieve these objectives. I also intend to look for different sources of evidence-based data that I can use to increase my knowledge and skills in these areas of practice.

Challenging Patients

I have experienced several challenging clients in the practicum experience. One of the clients is a 48-year-old male patient that was being discharged after being treated in the facility for substance use disorder. He was a challenging client because he lacked housing and resided in homeless shelters in the region. His situation implied that he was highly predisposed to recurrent substance abuse. The patient was linked with the existing social support services in the community. The second client is a schizophrenic male client that was brought as an involuntary admission. The patient was at risk of self-harm and others. The challenging part in his treatment was that he was irritable, aggressive, and declined treatment. The last patient is a 33-year-old female that had been diagnosed with post-traumatic stress disorder and lived alone. The patient was suicidal. Discharging her home increased the risk of her self-harm. As a result, she was admitted for monitoring despite her being against it.

I have learned several things from my encounters with different patients. Some of the lessons learnt include the importance of interprofessional collaboration in the provision of psychiatric care, use of evidence-based interventions, and the adoption of patient-centered care interventions. I have also learned about the importance of patient safety culture for the realization of optimum outcomes in psychiatry  (Moyo et al., 2022). The resources that have been available for use in the practicum site include our preceptors, supervisors, physicians, other nurses, and classmates. The evidence-based practice used for the patients include diagnosing them based on DSM-5, developing evidence-based treatments, and interprofessional collaboration (Haefner & Filter, 2022; Zhang et al., 2020). I will focus on using different screening tools to build my competencies. The hospital manages patient flow and volume through triage and scheduling appointments.

Communicating and Feedback

I will improve my skills and knowledge by being actively involved in all aspects of patient care. I will also seek support from my preceptor and other healthcare providers in the practice site. I have received informal feedback from my preceptor about the areas that I need to improve. I will incorporate them into my personal development plan.

Conclusion

In summary, I have achieved most of the objectives through the practicum experience. I have experienced challenging clients during the practicum duration. We utilize evidence-based interventions in patient assessment, treatment, and evaluation of outcomes. I intend to maximally utilize the resources in the practicum site to develop my desired competencies.
