When dealing with marginalized communities such as the homeless experiencing financial difficulties and restricted employment options, their ability to obtain quality healthcare and nourishing foods is undermined. This disparity in economic resources directly translates into diminished health outcomes, with higher susceptibility to chronic conditions like heart disease and diabetes.

The social and community context, including exposure to discrimination and violence, perpetuates stress and negatively impacts mental health. These determinants contribute to health disparities and the unequal distribution of health resources, leading to poorer health outcomes for marginalized communities (CDC, 2022). The Public Health Professionals Gateway also emphasizes addressing SDOH to improve population health. In my nursing practice, I could use this information to advocate for holistic patient care by considering patients’ social and economic backgrounds, collaborating with other healthcare professionals to develop interventions that address SDOH-related barriers, and participating in community health initiatives to promote health equity by addressing root causes of health disparities.
In collaboration with relevant stakeholders, the nursing profession can undertake multifaceted social justice and advocacy actions to address health inequities stemming from various social determinants of health. Firstly, nurses can work with community leaders and policymakers to promote economic stability by advocating for living wage policies, affordable housing initiatives, and job training programs (University of Kansas, 2021). Secondly, partnerships with educators can enable nurses to support equitable access to quality education through school-based health services, health education curricula, and mentorship programs.
Thirdly, nurses can enhance healthcare access and quality by advocating for universal healthcare coverage, increasing culturally sensitive care training, and expanding telehealth services in underserved areas. Furthermore, collaboration with urban planners and architects can help nurses advocate for safer neighborhoods, parks, and recreational spaces, addressing the built environment’s impact on health (Healthy People 2020, 2020). Lastly, forging partnerships with local organizations and social workers can aid nurses in addressing social isolation, promoting mental health resources, and organizing support networks, thus tackling the broader social and community context that affects health outcomes.
Nurses identify and address SDOH that impact vulnerable populations. One such specialty is community health nursing (CHN), where nurses work with at-risk populations like homeless individuals. For example, nurses serving homeless populations address issues like lack of stable housing and limited access to healthcare (Schoon & Porta, 2022). The significant SDOH affecting this population are economic instability and healthcare access. Economic instability leads to homelessness, preventing access to necessities and contributing to poor health. Limited healthcare access exacerbates health issues due to delayed or inadequate medical attention. These SDOHs collectively deteriorate the health status of homeless individuals, leading to chronic illnesses and reduced life expectancy.
In my practice as an upstream-focused community health nurse, I would engage in social justice and advocacy actions to mitigate the negative effects of SDOH on homeless individuals. This could involve collaborating with community organizations to provide housing and employment assistance, advocating for improved healthcare services tailored to the homeless population’s needs, and promoting policies that address poverty and inequality (Schoon & Porta, 2022). By working collaboratively with local government, non-profit organizations, and other healthcare professionals, I would strive to reduce health inequities experienced by the homeless, aiming to create a holistic support system that addresses immediate needs and underlying SDOH to improve their health outcomes.
