Discussion Questions

Topic 5 DQ 1

Quality and safety improvement initiatives in healthcare aim at enhancing the health and wellbeing of the diverse populations. Nurses and other healthcare providers assess the needs of their populations and implement evidence-based initiatives to address them. Several quality improvement initiatives exist in healthcare. An example of such a program of interest in this project is antimicrobial stewardship. Quality improvement initiatives can be used to advocate for antimicrobial stewardship. The aim is to ensure the safe antibiotic prescribing by healthcare providers and their use by the patients. Antimicrobial stewardship adopts the strategies of quality improvement such assessing things that need to be improved in antibiotic prescribing and use, implementing improvement strategies, monitoring, and measuring outcomes. The ultimate aim of antimicrobial stewardship is to ensure patient safety, lower the development of antimicrobial resistance, and Clostridiole difficile. The success of antimicrobial stewardship quality-improvement initiatives depends largely on the adequacy of support and infrastructure in an organization (McGregor et al., 2021). Therefore, nurses and other healthcare providers should be actively involved in advocating for change in their organizations.

Translational research can influence the development of quality improvement initiatives on antimicrobial stewardship. One of the ways it can influence is by guiding the processes adopted in the quality improvement project. For example, it may be necessary for the organization to implement the initiative in small-scale as a pilot study to determine its impact. The piloting will provide insights into the strategies needed to deliver the expected outcomes in antibiotics prescribing and use in an organization. Translational research also informs the strategies adopted to ensure the ethical nature of the quality improvement initiative (Ahlin, 2019; Thompson & Schwartz Barcott, 2019). For example, it informs the need for the consideration of ethical principles such as beneficence, justice, and non-maleficence in the implementation of the initiative.

Topic 5 DQ2

The practice site implements quality improvement initiatives to optimize care outcomes for its populations. A quality initiative adopted recently in the organization to improve outcomes is the incorporation of telehealth into chronic disease management. The organization adopted telehealth for the management of conditions such as heart disease, depression, and chronic obstructive pulmonary disease among others. The aim of the initiative is to reduce healthcare costs for the patients and organization as well as reducing complications in the disease management process.

The implementation of the above quality improvement initiative experienced some barriers. One of them relates to the legal frameworks associated with the use of technologies in healthcare. The organization had to adhere to the guidelines developed to ensure data integrity and protection of the patients’ rights, which delayed the implementation process. The other barrier was the lack of preparedness among healthcare providers. Most of the implementers had limited knowledge and skills on the use of telehealth in chronic disease management, which increased the need for training them before the implementation process. The other barrier was resistance from some healthcare providers (McArthur et al., 2021). The resistance was attributed to their perception that the change increased workload for them.

Translation of research into practice may also experience some common barriers. They include the lack of knowledge and skills among healthcare providers, inadequate institutional support, and lack of systems and processes that support the change. The other barrier is organizational culture that may not support change initiatives (Correa et al., 2020; Shayan et al., 2019). Therefore, nurses and other healthcare providers should develop anticipatory strategies to manage these barriers.
