Medication errors form a critical issue in healthcare. Medication errors predispose patients to unintended harms. In relation to the provided care scenario, the two nurses should not be disciplined. The most appropriate approach to addressing the issue would be investigating the factors that led to the medication errors. Often, medication errors arise from factors that include organizational, patient, and provider factors. Organizational factors include those related to the adequacy of the nursing staffs and the efficiency of the adopted systems and processes to guide medication processes. The patient factors include those related to the condition, workload, and changes in the health status of the patients during the process of medication administration. Provider factors may include aspects such as lack of knowledge, burnout, and stress (Asensi-Vicente et al., 2018). As a result, it is important to investigate the factors that led to the error. The factor could be those beyond the control by the nurses. The nurses should be disciplined in the same way should such a decision be considered. Medication errors can cause or fail to harm the patient. As a result, the measures of disciplining them should be the same to promote justice, fairness and equality in the organization.
